Need health care coverage? Enrollment Ends 2/15/2015 #USFGETCOVERED!
Getting health care coverage is about taking personal responsibility for your health and financial well-being. It’s about being an independent adult and protecting yourself. It is important to remember that open enrollment ends Feb. 15.
USF Navigators will be on-site for “Nav-Lab” enrollment events, providing enrollment assistance for the Federal Health Insurance Marketplace at the following locations:

Additionally, the USF Navigator offices will be open Feb. 14 and 15 from 9 a.m. to 7 p.m. to provide enrollment assistance.

If attending Nav-Lab or visiting a Navigator for an individual appointment, please bring the following information with you:
- Social Security numbers or document numbers for legal immigrants (including Certificate of Naturalization and/or Certificate of Citizenship)
- Employer and income information for every member of your household who needs coverage (i.e.: pay stubs and/or W2 forms)
- Policy numbers for any current health insurance plans covering members of the household
- Information about job-based coverage for all members of the household
